St. Anthony’s blended peer support case management program offers comprehensive services to help people build and maintain stability over time.
“Reaching full stability is oftentimes impossible without a community and the partnership of a Companion who, with lived experience, knows the way.”
– Companionship Guest

How it works
Multi-year: Coaching, support, and connection to resources, including targeted support to reduce triggers and setbacks
Grounded in Lived Experience: Our full-time Companions have lived experience in substance use disorder, mental illness, and/or being unhoused, and found success at St. Anthony’s
Comprehensive: Assistance with all ten elements of stability, including employment, housing, and medical care
Individualized: Our guests design and achieve goals that are unique to their personal needs, and we support their plan
“things get better when people believe in you.”
– Rahim“It’s not just about getting a job or a place to live. It’s about addressing the whole person—mentally, physically, and spiritually.”
